Step 3 - Fax them to 609-278-2650 or mail to: John Cooper NJ State Library PO Box 520 Trenton, NJ 08625 Step 4 - Then download the latest Training Information. Step 5 - After faxing the forms, e-mail Scherelene Schatz for JerseyCat log-in information.

Participation Agreement - Public Libraries  and Academic Libraries must have directors sign this form ( Schools and Private Libraries need only the Librarian to sign)

Local Library Profile Form -All Libraries must fill out. (If you have Z39.50  Technology and have a library collection of over 100,000, fill out the Z39.50 section)

Local Library Union Database Form - If you do not have a library collection over 100,000 and Z39.50 Technology you will need to fill out this form. (Fill out as much as you can)
